Passport/Pan card/voter id/licence [any one]

Incase of unavailability of ID proof, please submit your college id proof. However, this is temporary arrangement and you need to submit the ID proof once you get a valid ID such as PAN card or Passport

1)All academic marksheets  X,XII,8 semester marksheet,consolidation&provisional marksheets

2)Pan card

3)Birth Certificate

4)Medical certificate(photo should be attested by doctor)

5)12 passport size photoes(Take some extra photoes)

6)NSR acknowledement copy
